What is atopic dermatitis and why does eczema flare up?

Atopic dermatitis is a long-term inflammatory skin condition. It makes the skin barrier weak. Moisture escapes easily, while soap, dust, sweat and other irritants enter the skin more easily.

The result is dry and sensitive skin that becomes itchy or inflamed. Symptoms may settle for some time and then return again. This return of symptoms is called a flare-up.

Bangladesh has long periods of heat, humidity, rain and heavy sweating. Sweat trapped under clothing can irritate sensitive skin. During winter, lower humidity may make the skin much drier. Sudden temperature and humidity changes can also affect eczema symptoms.

Common eczema triggers include:

  • Hot weather and heavy sweating
  • Dry winter air
  • Harsh soap or body wash
  • Fragranced skin products
  • Detergent left on clothes
  • Dust and air pollution
  • Wool or rough synthetic fabric
  • Frequent handwashing
  • Stress and poor sleep
  • Scratching the same area
  • Skin infection
  • Certain workplace chemicals

Food is not the cause of every eczema case. Children should not be placed on a strict elimination diet without medical advice. Unnecessary food restriction may affect nutrition and growth.

Common symptoms of atopic dermatitis

Eczema does not look the same in every patient. Age, skin colour and repeated scratching can change how the affected skin appears. In babies, eczema often develops on the cheeks, face or scalp. Children may have patches around the elbows, knees, wrists, neck and ankles. In adults, it commonly affects the hands, face, eyelids, neck and skin folds. Common symptoms include:

  • Dry and sensitive skin
  • Frequent or severe itching
  • Red, brown, grey or darker patches
  • Rough or scaly skin
  • Small raised bumps
  • Cracked or peeling skin
  • Thick skin after repeated scratching
  • Fluid or crusting when infection develops
  • Burning or soreness
  • Disturbed sleep due to night-time itching

Atopic dermatitis (eczema) treatment we offer

Treatment depends on whether the eczema is mild, moderate or severe. Age and body area also matter. A cream suitable for thick skin on the hands may not be suitable for the face or eyelids. Below are the treatments we offer:

Skin examination and diagnosis

Skin Examination and Diagnosis

We check the rash, affected area, skin dryness and itching pattern. He also rules out fungal infection, scabies, psoriasis and contact dermatitis before starting treatment.

Regular moisturiser and skin-barrier care

Moisturiser is one of the main parts of eczema care. It helps trap water in the skin and supports the damaged skin barrier.

Topical corticosteroid treatment

Topical corticosteroid treatment

Topical corticosteroids reduce redness, itching and inflammation during an eczema flare. They are available in different strengths.

Non-steroid medicated creams

Topical calcineurin inhibitors, such as tacrolimus or pimecrolimus, may be considered in selected cases. These medicines can help control inflammation.

Treatment for infected eczema

Treatment for infected eczema

Repeated scratching breaks the skin. The doctor may prescribe an antibacterial treatment when there is clear evidence of bacterial infection.

Wet-wrap treatment

Wet-wrap therapy may be used for some widespread or difficult flare-ups. Damp and dry fabric layers are then placed over the skin for a limited period.

Phototherapy for difficult eczema

Phototherapy uses a controlled medical form of ultraviolet light. It is considered when eczema affects a large area or does not improve enough with topical treatment.

Phototherapy for difficult eczema
Systemic treatment for moderate or severe eczema

Systemic treatment for moderate or severe eczema

Moderate or severe atopic dermatitis may not respond to moisturisers and topical medicine alone. A dermatologist may consider systemic treatment.

Follow-up and flare control

Eczema can return after improvement. Regular follow-up helps adjust medicines, control new flare-ups and prevent long-term skin damage.

How atopic dermatitis is diagnosed

There is no single blood test that confirms every case of eczema. Diagnosis mainly depends on the skin examination and the patient’s history. The skin may also be checked for fungal infection, scabies, contact dermatitis, psoriasis or bacterial infection. These conditions can sometimes look like eczema but need different treatment. During the consultation, the doctor may ask:

  1. When the itching started
  2. Which areas are affected
  3. Whether symptoms come and go
  4. What makes the rash worse
  5. Which creams were used before
  6. Whether sleep is disturbed
  7. Whether asthma or allergy runs in the family
  8. Whether anyone at home has a contagious skin problem

Allergy testing is not required for every patient. It may be considered when the history points to a specific allergy or when usual treatment does not control the symptoms.

Eczema treatment for babies and children

Babies and young children can be more sensitive to some topical medicines, so eczema treatment needs to be selected according to the child’s age, affected area and severity. Parents are usually advised to keep the routine simple. Use lukewarm water, a gentle cleanser and a suitable moisturiser. Keep the child’s nails short. Soft cotton clothing can also reduce irritation.

Do not use an adult’s steroid cream on a baby. Do not mix several creams together unless the doctor has explained how and where to apply them. Medical review is important when:

  • The baby is feeding poorly
  • The rash is spreading quickly
  • The skin is wet, crusted or painful
  • Fever develops
  • Itching prevents regular sleep
  • Home treatment is not helping

When should you see a dermatologist for eczema?

Arrange a consultation when the rash keeps returning or does not improve with basic care. Urgent assessment is needed for fever, rapidly spreading redness, significant pain, widespread blisters, or a patient who appears unwell. Seek medical advice if:

  • Itching affects sleep or school
  • The rash covers a large area
  • The face or eyelids are involved
  • The skin becomes painful or infected
  • A child scratches until the skin bleeds
  • Previous creams no longer work
  • The diagnosis is uncertain
  • Eczema affects work or daily activities
  • Strong steroid creams have been used for a long time

FAQs about eczema treatment in Bangladesh

These FAQs cover common concerns about eczema symptoms, treatment, triggers and daily skin care. They can also help patients understand when it is time to consult Dr Siraj for proper diagnosis and treatment.

Can atopic dermatitis be cured permanently?

There is currently no single permanent cure for atopic dermatitis. It is a chronic condition, and flare-ups may return. Still, the symptoms can often be controlled well with regular moisturising, correct medication and trigger management. The aim of treatment is to:

  • Reduce itching
  • Settle active inflammation
  • Repair the skin barrier
  • Prevent infection
  • Improve sleep
  • Reduce the number of flare-ups
  • Help the patient return to normal daily life

What should eczema patients avoid?

Not every patient reacts to the same thing. Still, a few habits commonly cause trouble. Try to avoid:

  • Scrubbing the affected skin
  • Very hot baths
  • Fragranced lotions and oils
  • Strong antiseptic liquids on eczema
  • Fairness or bleaching creams
  • Random combination creams
  • Repeated use of pharmacy steroid mixtures
  • Tight or rough clothing
  • Picking at crusts
  • Stopping treatment as soon as itching falls

Which cream is best for atopic dermatitis?

There is no single cream for every patient. Most treatment plans include a suitable moisturiser. Active inflammation may need a prescribed steroid or non-steroid medicine. The right product depends on age, severity and body area.

Can I use steroid cream every day?

Use it only according to the doctor’s instructions. The strength, amount, body area and duration all matter. Unsupervised long-term use is not recommended.

Can atopic dermatitis affect sleep?

Yes. Itching often becomes difficult at night and may disturb sleep. Poor sleep is one reason to seek proper treatment rather than continuing only with home remedies.

Is eczema treatment safe during pregnancy?

Several treatments may be considered during pregnancy, but the plan should be reviewed by a doctor. Do not continue or stop a prescription medicine without discussing it first.
